John McCall 7133505936 Unify the BlockDeclRefExpr and DeclRefExpr paths so that 
we correctly emit loads of BlockDeclRefExprs even when they
don't qualify as ODR-uses.  I think I'm adequately convinced
that BlockDeclRefExpr can die.
